BHS Physicians Network wants to make your visit and relationship with your doctor as easy and convenient as possible. Please review the resources below.

We want your visit to be as pleasant as possible. Please see the information included in this section to prepare for your visit and to answer any questions you may have. Should you have any additional questions, please do not hesitate to call.

If you are a new patient, we ask that you download and complete the forms included in this section and bring them to your visit. Also, please arrive 30 minutes prior to your appointment in order to allow adequate time for the registration process.

Although we will strive to see you as close to your appointment time as possible, please understand that our cardiologists do have emergencies from time to time that could cause a delay.

What to bring with you for your visit

Please complete the following form and bring it with you to your first visit.

  • Medications in their original containers
  • Any test results that are obtained from the referring M.D.-examples:
    • EKG
    • Holter monitor
    • Stress test results
    • Letters
    • Past medical histories
    • Any logs or journals you may have been asked to keep
  • Any allergies to medications
  • Insurance cards
  • Referral for treatment from the primary care M.D.
